The two-page form was introduced in 1924 and required extensive personal information about the student as well as details regarding the courses they intended to take; it had to be filled out by hand multiple times each semester based on the course catalog and submitted. The form remained largely unchanged until the transition to computerization in 1967/68.
These forms remained within the university administration, but the same information was also found in the “registration book,” which remained with the students.
